We're in the process of standardizing facility addresses in eHARS and are looking for suggestions.  When facilities change addresses, are other jurisdictions creating new facility entries with the address and keeping the old facility entry separate, or changing the address?  Some of our facilities have also changed names, how are these handled?

In Virginia, we change the name in the eHARS table and update an excel spreadsheet to show current and previous provider name and/or address. We maintain a Facilities List as part of our regional Epi Consultant Training Manual.
